It's all pretty much explained in the movies.
Correct me if I'm wrong but:
1) City sewers and tunnels.
2) the whole thing started with a war of extermination between man and machine and the fighting is just a continuation of that.
There are humans in Zion that were never in the Matrix. Those would be the decedents of the people that had been fighting the machines for generations. And if humans in the matrix are their power source, why would the machines let them escape?
3) Fake existence makes them complacent. Cant even consider being human batteries = less chance of revolt.

No, they're just descended from people who escaped the Matrix. Zion had been destroyed dozens of times before the start of the series. They'd been going at it for thousands of years.
