Free standing ERs are a money grab in Texas, not sure why one was proposed for North BR.


The thought being the Ambulance not wanting responsibility for the med. during the hour infusion. This ER is major hospital affiliated though. The other I work is not. tPA is not stocked at this one. We would call 911 and GTFO. CT the head of course so that step would be handled on arrival to ER.

I'm at a tertiary center and our number of tPA transfers have gone way up in the past year or so. Most given at rural-ish EDs where they don't really know what they're doing and the complication rate is much higher than at a proper stroke center. I wish they'd CT and GTFO (even the CT is probably not necessary as we usually repeat it).
