No doubt that Human factors were a cause. But don't forget that multiple mechanical failures occurred all due to humans deciding to continue to operate equipment knowing that the equipment had mechanical problems.

No doubt. Bad cement job (caused by mechanical issues) led to communication between the reservoir and the surface.

But all of that would have been detected (really was detected) in the negative pressure test that was ignored. It looks to me like groupthink and go fever set in at that point.
