The report was from the Los Angeles Sheriffs Department Office of Inspector Geberal and is a blatant attempt to shuffle liability onto the gun manufacturer.
FACT - several of their employees Drew and pointed a gun at someone before putting their finger on the trigger and pulling it. The gun performed as designed and fired a projectile which struck the target. Now they want to claim that this is the guns fault because apparently it should know when you mean to shoot and when you don't. Not sure how that would work.

Exactly. The guns worked as designed. The officers were just horribly trained and a manual safety will not compensate for bad training.
