This is a good question.
Hill was the starter put of camp, and Lewis never publicly said Bernard was the starter. But Bernard gets double the snaps Hill does, and is usually on the field before Hill.
Maybe it goes by the official team chart, which Lewis never changed.

I would assume it's who is on the field for the first snap, but there is no way to check that, other than going back and watching the first snap of every game...
