You sound knowledgeable on the topic and my want (using a Les Miles-ism) is to believe you, but I suspect your knowledge is tainted with incentive. You sound like an insurance company man, am I right?
BTW, I too have experienced exactly what is being discussed. The "out-of-network" dentist rooked me bad.

I'd rather pay extra to go to a private practice than go to a corporate run place.
