The northmen are sent to arrest Ramsay after he marries a northern lady for her lands and locks her in a tower, leaving her to starve. Just before they catch up to him, he tricks Reek into assuming his identity and swaps clothes with him. Reek gets killed while pretending to be Ramsay, and Ramsay is arrested while pretending to be Reek, fricking a pig while covered in shite.
They bring "Reek" back to Winterfell as a prisoner just before Theon takes the castle. Afterwards, Theon frees "Reek" and thinks he's won a friend, so he lets him go to raise an army to defend Winterfell. Reek/Ramsay instead brings back an army of Bolton men, kills Ser Roderick after shaking his hand and pretending to be allies, takes WInterfell and that's when Theon's troubles begin.
It's a really brutal scene in the books, and is the first real indication that the Boltons are traitors.
