3x a week for 2-2 1/2 hrs. Split in to stations and go for 20 minutes per station. Take a 5 minute break between rotations.

Indi stations:
batting (soft toss & tee work)
infield ( grounders and situations)
Outfield (fly balls and situations)

Group practice. (20 minutes):
Work infield and outfielders (making sure all know their responsibilities for their position) and go through situations.

Break into teams of 3 and do live pitching and hitting. 9 in the field and 3 bat until they get 3 outs and then rotate.

End practice.

eta: if your son is 12 and your middle school has a baseball team I suggest your son starts swinging a -3 (drop 3) this season. Going from 50/70 infield to 60/90 is huge and i have seen some kids struggle.
As 1 poster mentioned on this site many months ago:

60/90 baseball is a dream killer.
-3 bats is the Undertaker.

Don't swing a -3 bat at 12? That's stupid. You're body will be ready for a drop 3 when it's time. A lot of physical maturity happens between 12 and 14-15, when you'll need to swing a -3. My son used a -8 at 12, -5 at 13 and used a -3 the fall before HS, perfect progression. Why screw up your 12 yr season swing at bat you can't get around!

The next step after 50/70 is 54/80 so you're not going straight to 60/90.
