Having to wear scrubs so much of the time makes things problematic. Pretty much rules out the usual methods of cc, outside waistband, inside waistband, other methods of belt carry and shoulder holster; few folks who start w/ an ankle holster continue to use it long term. Pocket carry is also a problem w/ scrubs. In my experience, the pockets in scrub suits are too shallow so that "things" frequently fall out of the pockets when one sits down; this holds true even w/ thigh pockets. Belly band type device (or those t shirts I've seen that have a holster sewn into them) are feasible. I've never used it and am basically uneasy w/ it, but "Smart Carry" might be an option, as well. Can you wear a "fanny pack?" As far as guns, my first rec would be a single stack 9mm (like the Shield) if you can comfortbly carry it. If not, I'd look at a pocket 380. In this situation, I'd personally, feel more comfortable w/ a a gun has a double action only type action and that an external safety lever. A lightweight snub nose w/ +P 38 Spec is also an option, as long as you learn the manual of arms adequately.

They make a tactical holster that now goes under scrubs, warmups, and the like. Goes around your whole waist, no need for a belt.
