If you are doing limit hold em, it really doesn't matter what the max is because you can't bully people around with big chip stacks. If you are doing 1/2 NL, 200 as the max and 20 as the min. 2/5 have 500 max, 40 min. This is how most casinos do it at least. If people want to buy in low and still play, that's fine...they just need to realize what they are up against.
ETA: In cash games, I've never pulled someone's blinds. Let them take a piss break or smoke break. Tournament style, you definitely take the blinds away.
