Still, what happens if he runs and loses? Does he risk his Senate spot? Unless Kentucky's House approves a change in the law he can't run for both Senate and President. So, does he risk it?

He just can't run for potus in Kentucky is one way around it.
Paul always stated he wants to go back to medicine after he's done in D.C.
It's now or never and to be quite honest this country doesn't have that much time left.
