Great episode - fantastic monsters, awesome CGI for a TV show, and really well written script. Capaldi nailed it as always, fast becoming one of my favorite Doctors, loved it.
I have no clue what's going on with Missy and I love every second of it.

A couple more thoughts this morning (haven't rewatched this one yet):
I don't think they're ready to go there yet, but this would have been a great use of "everybody lives" at the end, if he had figured how to save those in the walls. The last couple of episodes indicate that he really does care, but feels he needs to focus on making the hard choices. We have been led to believe at times that he doesn't mind a few here and there dying if it helps him find the solution. One of the main themes being "am I a good man?", they won't resolve this before the season finale, at the earliest. That could potentially a great "everybody lives" moment if he somehow saves those sent to "heaven" this season by the doctor's action or inaction. It might lessen the earlier episodes but it would be a great moment.
