Well we know for sure that the doctor intentionally set up that guard and tried to frighten him. This was immediately preceding that scene and I think gave a clue. Then Clara unintentionally became the monster under the doctor's bed. I do hope they don't try to definitively explain all of the happenings in the future.
Well she to survive long enough to have a kid, anyway.

Not necessarily.
95% sure. He's been in Cardiff (actually saw him Sunday), he's been with the DW people, and he's been seen in a Santa-like costume.
