I don't know a damn thing about Ultron, but I think it is in the slight realm of possibility that Tony Stark and Captian America come to blows over the creation of Ultron at the beginning of the movie and then Ultron kills CP towards the end.
I guess you would have to set up Bucky taking over as CA prior to CA3 though

No. Cap 3's story is already about Cap still looking for Bucky.
Cap will not die in Avengers 2. Neither will Thor. And Hulk, There are too many rumors around Hulk for that to happen.
Iron Man and Widow are the safest bets.
Cap is on the list for A3's death toll though, and is probably the go to death for that film.
