I have never heard of anyone falling out of their boat while operating it either, but I know several incidents where people have been thrown from a boat while operating it!
If you operate a boat long enough in the skinny waters of South Louisiana, you will eventually hit some debris or other obstruction. Depending on your speed and the angle of impact, you and everyone else aboard could easily be ejected.
I once hit an unseen, unrecorded obstruction at WOT. I was thrown into the console so forcefully, it felt like I had run into a brick wall.
