The basics are avocado, onions, Roma tomatoes, Serrano chiles, cilantro, and for mine a bit of garlic. And lime juice of course as well as salt and pepper.
I normally use 3 avocados, 2 roma, 1/4 onion, 1 lime, just a bit of cilantro, 1 Serrano, and maybe 2 garlic cloves.
Crucial to mash avocados prior to putting the rest of the stuff in. Add tomatoes last.
My Mexican friends say roma tomatoes and serranos are crucial.

I typically do 4 ripe avocados, about 1/3 of a purple onion, about 3 cloves of garlic, 1.5 limes, cilantro, and 2-4 serrano peppers.
I hate tomatoes.
When I want to get freaky I'll use two limes, scale back the onion and garlic a hair, and toss in a finely diced Granny Smith apple. It's a fun treat.
ETA: Oh, and salt and pepper. Not too much salt.
