That's a great link but it appears Cunningham was in a punt formarion and the defense was ready for it.

I'm talking about lining up in shotgun and the defense thinks your going for it and lines up in a 5-2 formation and the ball just goes way over anybody who is ready for it.

This is typically exactly how it's done.

Yes that happens. Pats did it a few years back. Plenty of college teams have done it.
