In Dany's vision she sees what I believe to be Rhaegar and Lyanna giving birth to Jon Snow and he says "his will be the song of ice and fire"
So to me that means that the Song of Ice and Fire is at least not about R & L. I think its really about Jon Snow being the result of R & L but I can see where it may lead people to believe it means Jon Snow(Ice) ends up with Dany(Fire)

Pretty sure that vision is of Rhaegar and Elia, the baby he is referring to is Aegon. He says there must be a third head and that is the beginning of meeting with Lyanna to have Jon.

I'm almost positive it was Rhaegar and Elia with baby Aegon in that vision. If R+L = J, Lyanna would be near death after the birth...or on the express to death, anyway.
The many aspects represented by the title ASoIaF are what makes it so well-chosen. It could be about Jon Snow, the Targs and the Starks, the Others and the dragons, the sword Ice and it's eventual re-forging ( a very Tolkienesque touch if so) etc...
I think the title reflects the forging itself...people, civilization and the trials and betrayals, relationships and love...mostly that it takes these two destructive forces used properly to create steel, a substance stronger than the combined effects of both.
Goes back to Howard's Conan and the whole "mystery of steel"- we know GRRM a big fan - the truth is steel isn't power, flesh is power, but it takes steel to control it.
Something like that, anyhow.
