I seriously doubt he's dead at all. Ramsay is just trying to scare the piss out of Jon Snow and provoke him to anger and do something stupid. The only bit of truth in that letter is that he's captured Mance Rayder, and that Reek and "Arya" have escaped. The Boltons are just screwed without "Arya" being in their possession.

I still think the letter may have been written by Stannis himself with the help of Theon/Reek to draw Jon away from the Wall. Don't ask me what page it's on here ( 
) but there's some stark similarities in some terms used in that letter and some things Theon/Reek says.
