W9 Forms for Single Member LLC...EIN or SSN?

I am a Single Member LLC and I have always filled out W9's with my company name and EIN. Now I am seeing that the IRS wants you to put your name in the NAME box and your business name in the DBA box and then supply your SSN instead of EIN??? I thought the whole purpose of a single member LLC using the EIN was to not have your SSN on all these forms? I have just been doing it the old way and checking LLC, then the D option for Disregarded then entering my EIN. Any thoughts?

Can you post the instructions that you read that states this?

If you are filing as an S corp it makes it easier for the IRS to look at the paucity of livable wages paid to you, if any, versus the massive pass through/bonus exempt from FICA you may pay yourself and claim on your 1040.

ETA: read the instructions that come with the W9 form...it talks about what to do.

I just asked my CPA and he said he fills them out both ways. He told me to just keep my company name on the NAME line and choose LLC and then enter my EIN. He just told me to make sure I stay consistent on all my W9's.
