I originally thought this was for a 4 way switch, but that doesn't seem to be the case, as I don't see 2 more 3 way switches anywhere. The red and black wires toward the bottom are hot. What kind of switch would this be wired to for a fan with lights? Any help woud be appreciated.

Disregard. I haven’t had my coffee yet.

Any chance one of those could be for the fan speed?

ETA: I have a fan in my office. There are 2 switches, one is the light and the other is for the speed on the fan. I wired it several years ago, but I don't remember what color they are.

The fan is one of the newer ones that originally came with a remote. The remote went out, so I bought the switch, removed the receiver in the fan and wired directly to the 3 position speed switch.

What voltage do you have between the 2 wires?

Could have had a double pole switch sending 240V.

Turns out it was wired to use for a remote too. I can just cap off the extra hot wire if it's not needed.

Are you saying BOTH the red and black wires at bottom are hot? This would appear to be a standard 3-way switch with bottom black wire hot, red wire is the traveler (shouldn't be hot if not connected to the black), and the top black wire to the load (fan). Looks like a single switch box - if you can't find the second switch of the 3-way, you could always cap the red traveler wires and just use the 2 black wires connect to standard 2-way switch to control power to the fan.
