Well, there is a ton of research on this specific topic and the line demarcation is hitters seeing a pitcher the third time.

The specific factors going into it (fatigue, familiarity) may be weighted differently, but the effect is undeniable. It can be lessened with each specific pitch thrown more than 10% of the time, so starters with a bigger arsenal are less susceptible (but not immune).

A reliever that mostly relies on two pitches that was getting to the end of his pitch count and inning max probably shouldn't face a potential 1/1 draft pick a third time. And this is something that can be pointed out in real time. No hindsight needed.
