Back in the day, there were so many things guys (me being one if them) could do to gain weight, strength, speed, etc.
Nutrition wasn't dialed in nearly as well as it is today, so guys like linemen had to eat crazy amounts of calories to sustain and/or gain weight. Not to mention aftermarket products that probably held risks, unknown at the time.
Then, there are genetic traits passed on thru families.
Then there's stress. I would imagine he had lots of stress with a plate of responsibilities as full as his.

No matter the underlying problem, that's a young age to have the heart kill a great athlete and physician, husband, and father.
