I don't personally know a single person who's ever fired a gun in self-defense, but I have witnessed at least two accidental discharges.
I guess everyone needs to make their own evaluation. My logic is that the likelihood of ever needing to use my gun in self-defense is incredibly small. And of those situations, only in a small percent would I not have time to rack a round. That must be weighed against the likelihood of an accidental discharge resulting from mechanical failure or user error, which I perceive to be greater.

Good for, you are one data point among millions of others that have used a gun in self-defense.

That's all fine and dandy until your number is called. People probably feel the same way about keeping a fire extinguisher handy or batteries in their smoke alarms, they won't think they need them until they do.

If you had any awareness on how quickly these situations can develop and how poor your reaction will be under duress...

I carry a stock Glock 26 IWB, the trigger is completely covered by the kydex outer layer. There's nothing that's going to cause the gun to fire short of a deliberate human action. The chances I might need to use that gun quickly are vastly greater than the chance of a random mechanical failure.
