I think the answer regarding the one-eyed polar bear is deliberately left vague by the show, but all signs point to Danvers' son Holden dying in a polar bear attack by the one eyed polar bear, and Danvers feels bad for all the times she played with Holden and pretended to "attack" him with the one eyed polar bear stuffy.

This obviously implies that a ghost observed Danvers playing with Holden in this manner, and then possessed the one eyed polar bear to actually attack and kill Holden. As a joke.
