The plenum has insulation attached on the inside of the “box” - that’s likely not the issue - though I did watch one HVAC tech Y/T video some time back where the part of insulation inside the plenum had separated from the box and it did cause some sweating (leakage) at supply registers.
Check carefully around the boot of the supply register (ceiling side and room side) for any leaks or lack of insulation to make sure it’s properly sealed.

This!! I had same problem. Moved insulation and found water on top of register box. New zip tie and spray foam insulation the connection up the duct about 4”. Have now been doing everyone because I feel the AC leaking around the connection.
