The VA is staffed by unionized federal employees and it's virtually impossible to hold them accountable unless their behavior is outright illegal or past the point of egregious (and can be proved). Couple this with managers getting pressure from congress and the public/vets groups to provide better service. It's a combination for the disaster that the VA is.
