Totally unacceptable.
If I'm that service manager I'm telling him "OK, I'm sorry, must not be you. Is there a Mike Smith here? Mike Smith? Anyone??" Until he gives in and says "That's me."
Secondly, if someone ever tries to correct you that you need to address them as Dr, the best course is to fall over dying laughing, then look at them with a straight face "Oh shite, you were actually being serious!?! Yeah, that's not going to happen."
