Depends on where on the flange it is leaking. If it is a lower sidewall fitting and the leak is where the flange meets the tank, there could be a crack in the tank that would only be visible from inside the tank.

It also depends on the fitting. If it is a transition fitting, there could be a leak inside of the internals that could be replaced fairly easy.

Do you know if the tank was Stainless Steel or Polyethylene?

It is not leaking at the tank.
