Only two really stand out:
First time I had Doe's Eat Place (I was fairly young) - but subsequent times didn't live up to the first one I had.
Second was first time I had Doris Metropolitan. I was a little older, and have had it a couple times since but still doesn't match up to the first time.
I think there needs to be a rare culmination of circumstances to make a meal (not just steak) really stand out from the crowd. Everything from the execution and presentation of the restaurant, to your own personal mood, hunger, company, etc.
