Vanguard or Fidelity seem to do a pretty good job.

Mine is through American Funds. Pretty easy

My Roth and rollovers are at Vanguard, expenses matter. No load, low ER choices. I also have had accounts at Fidelity, no complaints.

Just about any major brokerage is fine. I have mine with Schwab, have never been disappointed. I'd just pick a major name and roll with it. If they do something that annoys you, transfer the account (not hard) to another major name. You'll find one that works for you.
I like Schwab because their phone support is always very good, even on major holidays. I've worked with both as an institutional investor and as an invidual. But really any big name in the business probably does fine, it's more a matter of what you like most.
