This article is on the money. Under ACA rules, the only companies that can offer major medical plans are Healthshare plans. For generally healthy people who just need catastrophic coverage they are a great option. Either pay cash for routine care or join a Direct Primary Care practice and your monthly spend is less than $300.
Most HealthShare plans are non-profit and a much better alternative for generally healthy people. I use Zion Healthshare and think it works very well for me.

Hate to tell you, but we are single payer. CMS sets the standard for care and commercial carriers follow their guidelines. For non-traumatic joint injuries, approval for an MRI requires 2 week Ibuprofen and 4 weeks PT. A lower extremity non contrast MRI bills around $2,000. If you pay direct cash, it's a little over $300. However, many of the hospital groups are no longer letting you pay cash if they know you carry commercial insurance.
Bingo. Most physicians are employed and they have to practice in accordance with their employers directives or they get fired. COVID proved that in a dramatic way. There are still a few independent practices left where you will find physicians who put the care of their patients first.

Direct Primary Care is a version of concierge that doesn't participate in insurance. Get a healthshare plan like Zion and find a DPC practice. Most practices dispense medication in clinic and have negotiated rates for labs and imaging. LINK
I don't post on here usually, but just saw this thread and had to comment. I can get a CT for $150-200 through my practice. Direct Primary Care partnered with a Health Savings Plan like Zion Healthcare runs as low as $175/month and way less out of pocket costs. Much smarter and easier, but people stay with their crappy employer insurance plans for some reason. This model is big in the rest of the country but only a few practices in Baton Rouge, link below to search. I happen to think Sasser DPC is the best.
