Are you trying to claim this is what we have or where we are heading? The lobbyists that control our government and wrote the ACA would never allow this.
I didn't say there weren't pre-selected winners in this system, nor that crony capitalism was absent from this arrangement. To your point, it would never have been allowed to happen otherwise. But it is absolutely where this was intended to take us, and it is happening at an alarmingly quick pace. The COVID fiasco served only to accelerate the rate of corporate/bureaucratic consolidation.

We have actual data that shows Obamacare failed horribly at its main goal: reduce healthcare costs
And this is the real problem with this legislation and the general understanding of its intent. Cost savings were never the primary intended effect. The ACA was designed to do several things:
1. Accelerate consolidation of the administration of medical care into a few large organizations but reducing physician/professional reimbursement (and thus influence over the delivery of healthcare) and "relatively" increasing reimbursement to facilities. This then places control of the system into the hands of corporate bureaucrats.
2. Subjugate those large organizations to centralized federal control by then having undue influence over the reimbursement for medical services.
3. Centralized big data collection by the government through implementation of universal EMR usage.
4. All of which ultimately leads to the real objective of a de facto single payer, government controlled healthcare system. Which is what they wanted from the beginning.
And it is working amazingly well at all of these things.

Allard in the St Germaine neighborhood for excellent classic french food and Parisian atmosphere
Septime for a more avant garde foodie experience...been twice, best meal of our trip both times (over several 3 star Michelin restaurants). Lunch is a perfect experience, and is semi-casual.

Armando al Pantheon is excellent. It is in a very touristy area, but don’t be fooled, it is the real deal for Roman classics.
Piperno is kind of a ritzier version of Armando. Traditional Roman menu.

Had the same problem for the same reason. Had the same surgery (subacromial decompression), but mine was an open procedure. While my day to day recovery was quite quick, I really wasn't able to get back to anything close to what I was doing before at the gym for about a year. And even then it was with some major adjustments in how I did things, with significant limitations in certain movements.
Talk to your surgeon. Then talk to a good trainer who understands the mechanics of this stuff.

Michael Moses, Kamran Khoobehi in NOLA
Anthony Stevens, Mike Hanneman, Taylor Theunissen in BR
All good surgeons

Hansbrough or Lopoo are both excellent pediatric surgeons. I trained with them and would let either of them operate on my child over any other pediatric surgeon in the state. Upp is a good surgeon as well, but I prefer his partners. They all work together.
re: In manhattan, saints bar?

The answer is Bar None. Only Saints bar in NYC I'm aware of, and its a pretty good time.

Another lie. Urgent cares and walk in clinics aren't legally allowed to prescribe opiates, benzos, adderall, or any controlled substance
Of course they are. Any licensed MD with a valid DEA number can prescribe these meds in a clinical setting.
Edit: I see that perhaps it may depend on individual state law.

It's called patient satisfaction.
You can thank Obama and his Affordable Care Act for that.
Many MDs/PAs/NPs are told to make the patients happy, and this means give them what they want.
This. And they're not just being told. Their reimbursement will soon directly depend on patient satisfaction scores. Chew on that.

Issue is the percentage of out of pocket scripts adjudicated at pharmacy. Off book, no insurance fills. Valid fills will refill too soon due to PBM benefit edits. In other words, an addict using insurance can use the insurance to legally fill only so often. The issue lies with addicts running from internist to internist or FP to FP due to "pain". There is no national database for doctors to refer to in terms of medical history per patient SSN. Use a different pharmacy for each Rx and it's invisible.
False. There is an easily accessible state-wide database that shows records of precisely which controlled substance prescriptions were filled by any individual patient. It shows exactly which prescription was given, the date it was attempted to be filled on, and which pharmacy the patient went to. It is used frequently by ER physicians who deal with drug seekers on a daily basis, less so by the family practitioners and internists.

I have 2 orchestra level (standing room only) tickets available to Wilco this coming Monday 9/28. Was excited to attend, but can't go now. Asking $150 for the pair. Message if interested.
