Gretna Medical Center LINK /
gives the test and gives you a document with the codes and stamps required for entry by some countries...Egypt in my case.
It's expensive $200 for the test and $50 for the document.
Many BTR and NOLA places give the test, some free. Haven't found any other place that gives you the mandatory document.

I just got back from Europe, so not sure if this answers the question or not since you're talking about Egypt.
Not sure an American in America can get a card with a QR Code, or get one overseas without the help of a physician or pharmacy? We got our EU Card only because the tour director called in some favors with a local pharmacy. They took our US Vax Card and entered us into the systems. Europeans use an app that's has the code if they're asked. Very simple to use. We had to present the paper version, and in most cases, they'd use the US card anyway.

Just returned from Egypt with transit through Frankfurt. QR code for negative PCR test was not required by Lufthansa or Egyptian medicals upon arrival. We used CVS services for testing and had everything printed as well as saved to our devices.
96 hours is the important part of the testing, they will not let you board unless the timing is correct!

If the question is how to get back into the US, that's a State Department requirement.
The test must be administered no more than one calendar day before the date of the international flight to the United States.

In our case, they came to the hotel for an antigen test. 1pm Saturday appointment for a 12:43pm Sunday departure. 15 minutes for results. $183 for 3 people. Cash only. Gave us a paper certificate for proof of negative test. They asked about it at the counter in Warsaw, but never again elsewhere.

Yes, we had our PCR test 24 hours prior to flying back from Cairo as well with no issues. We did have to provide the negative tests several times in Frankfurt before our flight back to the states.
Going to Cairo it’s negative PCR within 96 hours. The timing of the test has to be valid from wherever one is flying from in Europe or elsewhere. There were lots of folks who weren’t allowed to fly to Cairo as their tests fell out of the 96 hours and had to take a PCR in the airport and await a later flight. Lots of pissed off travelers.
