I honestly want to throw this thing into the nearest lake.

If you can be specific about the issue, hopefully a mac user can chime in and help (change your topic title to something to get their attention). Google produces tons of results for easily installing OS X from scratch.

It seemed like you were at the right screen to begin with and should've selected "Reinstall OS X" instead of disk utility, which is what my previously posted instructions said to do.

Initially when I did that there was no disk showing to install OS X to. That's where the USB came in.
I have yet to even see the SSD shown.
The guys at frys told me it should be formatted already. So it's just about getting OS X up and running.

That's the sort of details that help get to the root of a problem.
Regardless of whether or not it's formatted, the system should still see it. But the guy at Fry's is wrong anyway (no surprise there). If Disk Utility didn't see it, it's either not plugged in correctly or your cable is bad, which would likely be the source of your other hard drive failures.

But why does that cable keep going bad? I don't get it. I reset the PRAM like 3 times. Now it's frozen in the Internet recovery mode. It's just showing the globe with no movement.
It won't install onto the usb either.
And you're probably right about the frys guy. I should have bought the damn connector and not listened to that bastard. I need to install OS X to the Ssd THROUGH the USB I think.
There's a picture on the directions with the ssd package that shows connecting the ssd through the USB with it. Damnit so mad. Damnit.
I'm so damn mad about this shite.
Note to self: do not ever listen to frys employees.
Thanks for your help btw. Honestly.

So, maybe what you have there is a lemon. Almost 4 years old. Time to move on.

The latest :
Got a SATA cable, installed to SSD externally through USB, OS X booted externally just fine, working great.
Installed SSD internally, still not showing up on boot menu.
Google search said something about "enabling TRIM" and so I used a third party app to enable TRIM with ssd attached externally. cindari?
Booting up now with ssd internally to see.
Read something about the SATA cable being the issue . Wouldn't be terrible I guess.
SSD still not showing up with TRIM enabled.
Maybe it's the internal SATA chord.

I'd be surprised if enabling TRIM allowed the Mac to recognize it internally. I think your interface is broken. I'll bet your old hard drive works fine.
Your warranty service log shows that the cable was replaced, but I wonder if it's the truth. Maybe they just kept throwing hard drives in there and the log just shows the other stuff by default.
If it doesn't boot, my last pieces of advice are:
- Open the laptop again and disconnect the SATA ribbon cable where it attaches to your motherboard. Wipe it down with isopropyl alcohol and inspect it thoroughly (magnifying glass?) for nicks, tears, exposed metal, etc. If it appears undamaged, reconnect it (DRY) to the motherboard securely and try again. It could just need to be reseated.
- Buy a replacement cable from a place that takes returns. If a new cable doesn't solve the problem, I'd call it quits.

Well can't I just run it normally externally? It works normal just only as an external HD.

Interesting...so I disconnected the cable, couldn't find any isopropyl (not at home), decided to reconnect it anyways, rebooted and it's noticeably slower now.

Tried to reboot again, just froze at blank screen.
Detached the cable again and reattached, rebooted, now it's moving normal again.

I'm confused. Are you saying that your internal drive is working now that you reseated the cable?
As for running as an attached USB drive, you can do that, but USB 2.0 is limited to about 60 MB/s (usually much less), which is pretty slow for an OS drive. It bottlenecks even a regular hard drive. Basically eliminates the advantage of an SSD.

Sorry. No it wasn't. I meant it was moving normally through the Internet recovery steps.
I'm going to clean it with isopropyl and then get back to you.

Okay. No luck. Ordering a new SATA cable.
Posted on 9/28/15 at 2:26 pm to tiggerthetooth
I assume you know what to order and where to get it?

Well, yes, I have the number on the cable, but I was sort of being cautious because I want to make sure the cable will be compatible with an SSD. Do I just buy the same thing that is on there now?

Whats currently on there.
Some of them have the "HDD" in their description, and I'm not sure if that means its specifically for an HDD or not.

Nah, doesn't matter, interface is the same. Just wanted to be sure you got the right one. Wouldn't put it past Apple to change the motherboard connection pin-out with every single model.

Okay, ordered it w/warranty.
Whats been ordered
Should arrive within the next two days.
