We got a similar email ASKING us to complete the "survey"
It included the following 4 attestations....but it did not ask for any documentation.
I will not ask anyone about their vaccination status.
I will not harass or intimidate anyone about their vaccination status.
I will not harass or intimidate anyone about wearing a mask.
I understand that I may wear a mask regardless of my vaccination status.
My understanding is that legally - they can ask if you were vaccinated, but they cannot ask why you were not.

All you have to do is "identify" as vaccinated, thats how it works now, right?

Ours said masks are required, but the last paragraph says ‘If you’ve been vaxxed, you don’t have to follow policy.” and nothing about checking.
