I don't keep track of the retarded/ignorant scoreboard, so I'll take your word that some MDs have been subjected to that invective like everybody else here. I was actually responding to an MD (or med student) that was denigrating the intellect of those posting with ideas different from his.

Are their non-physicians who know more about cardiology than me? I’m sure there are. Are there non-physicians who know more about infectious diseases and the clinical manifestations of COVID19 than me, especially in this thread? I very much doubt it.

Very narrow window of efficacy if it works at all. The data is ok very early in disease, but 5-7 days out from symptom onset the evidence of benefit is minimal at best. Personally my sense is it doesn’t do much for most who get it given when they get it. It’s reasonably safe however, relatively rare liver toxicity and metabolites can cause an issue if your kidneys really suck (we generally avoid it at CrCl less than 20-30). It can also cause relative bradycardia but this generally asymptomatic and heart block is rare.

Acute renal failure with dialysis requirements in critically ill covid patients usually signals the point of no return. Surviving multi organ failure secondary to Covid is uncommon unfortunately, the hypoxia is just so profound that you aren’t adequately oxygenating your tissues.
