I don't know where this conception comes from, but the rate of transgender individuals in the US dovetails nicely with the rates of Disorders of Sex Development. In fact, if you include things like Klinefelter, the rates of DSD's would be alarmingly high. In the case where there is no karyotypic cause, the prevailing evidence suggests that early exposure to specific maternal hormones in embryo development can cause a 46XX individual, for example, to develop virilization of the brain (or lack thereof in 46XY individuals), which is why there is some evidence that the brain scans of transgender individuals correspond more closely to their desired gender rather than their assigned gender.

A review of sex differentiation effects on the human brain noted that one reason you can have sexual identification different from the karyotypic designation is due to the fact that the development of sexual organs occur before the sexual differentiation effects reach the brain. The available evidence using individuals with CAH and CAIS also supports this.
