Educate me on confirmations

So over the past 2 years I have heard and read that most of Trumps appointee's have not been confirmed. How is that possible when Republicans control the house and senate? I realize there are double agents in the GOP, but aren't there enough votes to just push through all of these appointees through?
Also what happens to the SCOTUS slot if Lib's take over after Mid Terms? Can they just deny deny deny until 2020 if they want to? And keep Trump from appointing and getting his nominee confirmed?

The senate has a lot of arcane rules and the Democrats use these rules to slow things down.
The Democrats are slowing things down but not enough to keep Trump from appointing conservative judges.

Another one is that Kavanaugh does not need positive affirmation from the committee. Grassley could stop this nonsense with a vote. If Flake abstains, Grassley can send the nomination to the floor.
