Did you know there was a Covid Form? DD Form 3150 "Certificate of Vaccination"

It was just sent to me from the company I work for. One of the options is "I decline to respond", which I would have chosen but I had the option of refusing to attest one way or the other which I did.
The instructions were to fill it out and keep it on you if you were going to enter a DoD facility.....so "papers please" comes to mind. Also fricking bullshite comes to mind.
