Flagstone should be level with the concrete near it. Box it in with 2x4's and put sand in there. Drag a long 2x4 across the top to get it perfectly level. Make sure you tamp the sand down or the limestones will begin to sink some. Take the time and do the hardwork first that way when it comes time to lay the stone all you're doing is dropping them in place.

Also, I think the 1st and 3rd pics look better IMHO. Keep it simple and space each stone out nicely.

I maintained the ones I installed for a few years. You can literally spray a weed killer on the area to kill broadleaf weeds (most likely to pop up) without harming the Mondo.
And as mentioned, it doesn't "spread", so weed eating is not necessary.
