Just completed the process. Rubber piece on sides was coming off and nose piece had started to degrade. Accessed Online customer service for warranty - answered questions and uploaded pics. 2 days later received an email that damage was covered with instructions to print a UPS label and send pair back. Lens were grey and I added a handwritten note to please replace with mirror blue. Replacement pair sent within 10 days. No issues - never talked to a person - online correspondence only.

They even agreed to swap the lens! Worked great for me.

I had a pair of Costa's that were about 5-6 years old. A friends kid back broke the frame back in October. Filed a claim and got a brand new pair back after a few weeks.
