I fired a .50 Cal machine gun without ear protection And pretty sure I have hearing loss. Not sure what you can do in certain situations.

just tape a water bottle to end of barrel and you can't hear anything.

Any good ENT office can make you some valved custom fit ear plugs. They are wearable for extended periods of time, fit inside the ear, and allow normal hearing with only a slight decrease in hearing for faint sounds. I wear them duck hunting and have troubles hearing wings cutting the wind at a long distance but can still hear em before I see em. The valve closes over a certain decible level and prevents further sound transmission and ear damage.

I worked with a guy that shot someone in his living room with a 357, he never mention how loud the sound was.
