my thoughts...
she was ambiguous on her boundaries. And, thats not your fault, but it is your problem now. You should tell her that you have thought about it and wanted to talk to her about her being upset about how you finished the other night.
Tell her that and made an assumption based on preconceived notions. And, that you realized after the fact that it was very unsettling to her. Apologize for how it upset her and let her know that you both decided to take things slow... and that you want to respect her wishes and only want to do things she is comfortable with. And, ask her what is in her comfort zone going forward.
This conversation will let her know that you didnt foresee it being an issue, show her that you are a good at communication and that the last thing you want to do is piss her off in that way. and it will give you a fricking idea of whats in and out of bounds so you can keep getting your dick wet.
