That is a difficult question to answer. A typical lab fee may run form $100 -$500 depending on material choice and quality of lab. A really high end lab may charge a fee equal to insurance UCR to make a single incisor in the esthetic zone (smile).

Go to a local dentist. It's happened to me before. It's not that difficult a job for a competent one to do.
