Did you have a reverse total replacement? If not see your surgeon and ask. Yeah yeah you don't want any more surgery but would you rather have this shitty shoulder forever or explore every option? If you did have a reverse I'm impressed you managed to frick it up.

I started smoking pot to help with my nerve damage. not saying it will help you. I was given 200 perks for the month cost me like 36 bucks with my insurance sold em for 5 a pill. bought some(lots) of pot and a ps4

If you aren't interested in surgery then a mix of therapy, injections, and prescription pain control (no don't take them daily but at least have them for an as needed back-up) may be your best bet. Try looking for a Physiatrist in your area (a doctor who specialized in Physical Medicine and Rehabilitation). They are kind of like an Orthopedist who doesn't operate and a lot of times are part of an Orthopedics group. They can usually help coordinate something like this or provide you with alternative options

Ligament reconstruction and the cut off some of the bone and such.
