In addition to it not being staged, Rosenthal has said that he thought he didn't get a decent photo at all.
He was preparing to take the photo of the second flag raising once it was all the way up. However, to get a better view, he stepped backwards right as the Marines started to raise the flag. He stumbled on a rock which caused him to hit the shutter accidentally, but at the perfect time. He then took other photos of the flag raisers posed around the pole, and sent the film off.
He thought one of the other photos was the one that was becoming famous when he was told that Stars and Stripes had picked it up. This puzzled him, because he didn't think they were anything all that out of the ordinary. Only after he got a copy of that issue of Stars and Stripes did he ever see the perfection of the photo.
It's a nice legend.
