Another thought, maybe the pain rears its ugly head when you are late? Pain could be getting worse and the medication masks it to a point. When it wears off it's bad. Give your dog medications on time.

My last dog was taking 2 tramadol morning and evening along with prednisone once a day for about 3 years before I had to have him put down. Never had any issues with him besides nasty gas.
