If a doctor is diagnosing a pilot with "possible psychosis" the doctor should be obligated to report and suspend his pilots license.

If you don't pass a physical to drive a truck you don't get a CDL.

It's the passengers' responsibility to do their due diligence on airlines and their pilots. No different than any other product.
