This drama queen would fall under the bolded part. Being in a same sex marriage constitutes an open and public rejection of Church teachings. Same as if he divorced and remarried or had multiple wives, except in the latter cases liberals wouldn't be knighting for him.

This covers most people. But if a person isn't openly defying the Church, then they're supposed to self-police. It's on the honor system. If you truly respect the Eucharist, you'll make sure to go to confession if you commit a mortal sin. If Jesus were coming to your house to visit, you'd clean it up before He arrived. Same principle applies here--clean up your soul before He arrives.
